Job summary

Community Action Partnership of Orange County is seeking a Program Coordinator-Housing Navigator (Case Manager) to provide case management services to vulnerable populations, help clients find and maintain housing, and coordinate support services to improve housing stability.

You will screen clients, create individualized housing plans, assist with housing applications, and collaborate with partners to secure and maintain housing across Orange County in a hybrid office/field/remote setting.

Qualifications

  • Bachelor’s degree in human services or related field.
  • 1–2 years in housing-related programs with case management experience.
  • 1 year of trauma-informed care, harm-reduction, domestic violence, and crisis intervention.
  • Bilingual English/Spanish preferred (written and oral).
  • Valid California driver’s license, reliable vehicle, and proof of active auto insurance.

Shift: Day (Hybrid schedule – office, field, remote work)

Salary: $25.00 – $27.00 per hour

Overview

Program Coordinator-Housing Navigator (Case Manager) will provide case management services to vulnerable populations, help clients find and maintain housing, and coordinate support services to improve housing stability.

Responsibilities
  • Provide comprehensive case management, including screening and assessing clients for housing navigation services and connecting clients to community resources such as CalAIM and Enhanced Care Management.
  • Create, implement, and monitor individualized housing support plans that address barriers and include measurable short- and long‑term goals.
  • Assist with housing applications, secure required documentation, and ensure housing stability plans progress as scheduled.
  • Arrange and support move logistics, schedule appointments, and perform related clerical duties.
  • Provide education and training on tenant and landlord rights, responsibilities, and housing stability best practices.
  • Advocate and link clients with community resources to secure and maintain housing.
  • Help clients acquire and improve self‑help, socialization, and adaptive skills necessary for successful residence.
  • Collaborate with staff, partners, landlords, and clients.
  • Participate in community events and supportive service events for those experiencing homelessness.
  • Enroll participants in the Coordinated Entry System.
  • Build a network of resources by connecting with landlords, housing authorities, housing navigators, lead care coordinators, and community partners.
  • Work collaboratively across departments to provide wrap‑around services, including CalAIM programs and ECM.
  • Attend agency team meetings and training as required.
  • Complete and submit program reports on a scheduled basis or as needed.
  • Use motivational interviewing, trauma‑informed care, and harm‑reduction approaches.
  • Perform other duties related to the housing navigation program as assigned.
Qualifications
  • Bachelor’s degree in human services or related field.
  • Minimum 1–2 years working in housing‑related programs, with at least 1 year of case management and case planning experience.
  • Minimum 1 year of experience in trauma‑informed care, harm‑reduction, domestic violence, and crisis intervention.
  • Housing navigation experience is a plus.
  • Preferred: Bilingual in English and Spanish (written and oral).
  • Valid California driver’s license, reliable personal vehicle, and proof of active auto insurance.
  • Willingness to travel throughout Orange County, including local business‑day travel and occasional evening or overnight travel.
  • Physical ability to lift up to 20 pounds and perform tasks such as bending, stooping, reaching, and walking in varying weather conditions.
  • Computer literacy: Microsoft Office (Word, Excel, PowerPoint, Outlook, Publisher, Access), web‑based databases, and virtual communication platforms.
  • Strong organizational skills, attention to detail, customer‑service orientation, and ability to work independently and as part of a team.
Travel & Physical Demands

Travel is required throughout Orange County; most travel is local during business hours from CAP OC locations, typically using a personal vehicle. Travel may also occur in the evenings or overnight. Physical demands include regular bending, stooping, reaching, pulling, pushing, standing, kneeling, sitting, twisting, turning, walking, and lifting up to 20 pounds. Candidates must be able to perform prolonged computer work and adapt to varying temperatures and weather conditions.
