Lee Hecht Harrison Nederland B.V. is seeking a compassionate Case Manager to support individuals experiencing homelessness across Central Orange County. The role involves outreach, intake, resource navigation, and housing stabilization services.

This position emphasizes building relationships, helping clients access housing resources, benefits, healthcare, and community programs while promoting self-sufficiency and independence.

Qualifications

• Experience in case management, homeless services, housing navigation, social services or related field.
• Strong communication, engagement and relationship-building skills.
• Ability to work independently in the field and manage multiple priorities.
• Knowledge of community resources and supportive services preferred.
• Strong organizational and documentation skills.
• Comfortable working with diverse and vulnerable populations.
• Valid driver’s license, reliable vehicle, and current auto insurance required.
• Mileage is reimbursed.

Responsibilities

• Conduct client intakes, assessments, and enrollments.
• Meet with clients in community and public settings throughout Central Orange County.
• Build rapport and establish trusting relationships with individuals experiencing homelessness.
• Connect clients to housing resources, benefits, healthcare, employment services, and community programs.
• Assist clients with completing applications and required documentation.
• Support clients as they transition into housing and work toward stability.
• Develop individualized service plans and track client progress.
• Maintain timely and accurate case notes and program documentation.
• Collaborate with community partners and internal support teams.
• Empower clients to achieve their personal goals while encouraging accountability and self-sufficiency.
