Job summary

Responsibilities

• Conducts comprehensive intakes and housing-focused assessments to identify barriers to obtaining housing.

• Coordinating with landlords and property managers to identify housing opportunities and advocate for participants.
• Maintains compliant, timely documentation aligned with Medicaid services and county standards.
• Provides ongoing housing stabilization and tenancy support to reduce homelessness.
