Job summary

CRI-Help is seeking a Housing Navigator to support Persons Experiencing Homeless in Los Angeles County. You will identify, apply for, secure, and move clients into permanent housing, while coordinating care with RBH or RH participants.

Under supervision, you will perform assessments, develop IHSSP plans, and link clients to housing, mental health, medical, and legal services. You will maintain documentation and participate in team meetings and trainings.

Qualifications

• High School Diploma or GED required.
• 2+ years of experience in housing and with diverse populations.
• 1+ year of Housing Navigation or Case Management experience.

Responsibilities

• Conduct comprehensive clinical assessments and provide intensive case management.
• Develop IHSSP plans with measurable goals and assist participants.
• Coordinate charting and documentation within program guidelines.
• Build relationships with agencies to access housing, mental health, medical, and legal services.
• Assist with housing applications and required documentation.
• Network with housing providers and landlords to expand housing options.
• Perform home visits and assess client needs regularly.
• Maintain HMIS accounts and link clients to Electronic Homeless Programs.
• Provide clinical interventions and crisis management when needed.
• Advocate for clients to receive entitled benefits.
• Participate in supervision, meetings, and trainings.
• Submit weekly/monthly reports and audits.
• Implement harm reduction and trauma-informed care consistently.
• Collaborate in a multidisciplinary team with open communication.
