Job summary

Qualifications
- Degree in social work or related field preferred.
- High school diploma required.
- Experience with mental illness, homelessness, or criminal justice involvement a plus.
Responsibilities
- Accurately complete and submit supportive housing applications (CAPS).
- Develop individualized treatment plans and document client services.
- Evaluate eligibility for temporary rental subsidies and furniture/moving allowances.
- Liaise with HPD, DHS, and HRA to obtain entitlements timely.
- Act as liaison between clients and landlords to support housing stability.
- Maintain audit-ready clinical charts in compliance with NYS OMH, Medicaid, and regulations.
- Assist clients in obtaining necessary documentation for benefits.
- Provide ongoing counseling and supportive services to achieve treatment goals.
- Schedule and accompany clients to apartment viewings and move-ins.
- Attend staff meetings, supervision, and training; perform other duties.
