Job summary

Fortune Society, Inc. is seeking a Case Manager to provide supportive services to formerly homeless adults, including seniors and veterans, residing in Brooklyn.

You will connect clients to mental health, medical care, legal services, and entitlements while conducting home visits and ensuring progress toward individualized treatment goals. The role requires crisis response, advocacy with housing and social service agencies, and meticulous documentation to comply with NYS OMH and Medicaid

Qualifications

• Degree in social work, psychology, rehabilitation counseling, or a related field.
• 1–2 years of experience with mental illness, chronic homelessness, or involvement with criminal justice.
• Strong communication, interpersonal skills, crisis management, and ability to work independently in the community.

Responsibilities

• Create and maintain client treatment goals and strategies for symptom management.
• Connect clients to mental health, medical, legal services, and entitlements.
• Conduct regular home visits to monitor living conditions and progress toward goals.
• Respond to crises, apply de-escalation techniques, provide on-call support.
• Advocate with landlords, legal systems, and social service agencies for client rights.
• Maintain audit-ready charts and treatment plans in compliance with regulations.
