Job summary

The Fortune Society, Inc. is hiring a Case Manager for Freedom House II in Brooklyn, NY. You will screen, assess, counsel, and link homeless and formerly incarcerated participants to transitional or permanent housing, while coordinating with internal departments.

Daily duties include intake, treatment planning, crisis intervention, and documentation. Travel to Fortune housing sites is required; daytime Mon-Fri schedule.

Qualifications

• High school diploma/GED required; some college preferred.
• Minimum of two years’ experience working with individuals impacted by HIV/AIDS, substance use, and/or criminal justice involvement.
• Experience serving clients, at-risk youth, and other disenfranchised populations.
• Strong organizational, follow-up, and leadership skills.
• Excellent verbal and written communication skills.
• Proficiency in Microsoft Word and related computer applications.
• Valid driver's license with a safe driving record.

Responsibilities

• Conduct intake interviews and needs assessments of newly released individuals and clients eligible for transitional housing services.
• Create individualized treatment plans for each client.
• Document all client services, including outreach attempts.
• Conduct crisis intervention with clients, as needed.
• Collaborate with Fortune Society departments to ensure quality care.
• Follow-up with referral agencies regarding client progress.
• Assist and prepare documents for permanent housing placement.
• Submit completed packets to the Housing Specialist.
• Perform and document room inspections.
• Mediate issues affecting clients’ health and housing stability.
• Assist clients in obtaining documentation for benefits applications.
• Complete and submit clients’ applications to benefits provider agencies.
• Provide ongoing counseling and supportive services to achieve treatment goals.
• Facilitate support groups and community meetings.
• Enter client services in the agency’s database and funder systems.
• Prepare and submit reports on client intake and discharge activities.
