Job summary

LaSante Health Center in New York is seeking a compassionate Housing Specialist to support patients facing housing instability, eviction risk, homelessness, and rental arrears. You will assess needs and develop action plans.

You will coordinate with landlords, housing agencies, shelters, and community organizations; help gather documents and complete applications; and track progress with therapists and case managers.

Qualifications

  • Experience in housing services, case management, social services, or a related field.
  • Familiarity with New York City housing resources.
  • Strong communication, organization, documentation, and follow-up skills.
  • High school diploma or equivalent.
  • Bilingual skills are a plus.

Responsibilities

  • Assess patients’ housing needs and develop action plans.
  • Assist with affordable housing, supportive housing, NYCHA, Section 8, shelter, rental-assistance, and eviction-prevention applications.
  • Help patients gather documents and complete applications.
  • Coordinate with landlords, housing agencies, legal providers, shelters, and community organizations.
  • Document all patient interactions, referrals, outreach, and follow-up activities accurately and promptly.
  • Track applications, deadlines, and patient progress.
  • Work closely with therapists, case managers, and medical providers.
