Job summary

The Fortune Society in Brooklyn, NY is seeking a Housing Specialist to work with justice-involved participants to secure permanent housing.

You will conduct screenings, develop individualized plans, and coordinate with HPD, DHS and HRA to help clients obtain entitlements and housing stability.

This full-time, non-exempt role focuses on outreach, documentation, and advocacy to promote successful community reintegration.

Qualifications

  • Bachelor’s degree preferred; high school diploma required.
  • Experience with mental illness, homelessness, or justice involvement preferred.
  • Strong communication and crisis management skills.
  • Valid driver’s license with a safe driving record.

Responsibilities

  • Accurately complete housing applications through CAPS; develop treatment plans and document services.
  • Evaluate eligibility for temporary rental subsidies and moving allowances.
  • Liaise with HPD, DHS, and HRA to ensure entitlements in a timely manner.
  • Act as liaison between clients and landlords to mediate issues and support housing stability.
  • Maintain audit-ready clinical charts in compliance with NYS OMH regulations.
  • Schedule and accompany clients to apartment viewings and housing interviews.
