Job summary

PATH is seeking a Case Manager/Housing Navigator in Los Angeles, CA to help clients move out of homelessness by building individualized service plans, coordinating housing goals, income supports, and health resources. The role emphasizes a whatever it takes approach within the Housing Navigation Team.

As a full-time, non-exempt position, responsibilities include intake, ISP development, referrals, and maintaining HIPAA-compliant records; driving is required.

Responsibilities

• Conducts intake and individualized needs assessment for all clients and develops an ISP addressing housing goals and related needs.
• Provides referrals, linkages, information, and support to resources to achieve ISP goals.
• Completes program-specific assessments (e.g. VI-SPDAT).
• Maintains documentation standards and HIPAA confidentiality for client files.
