Job summary

Pacific Clinics is seeking a CalAIM Housing Navigator to ensure timely housing services for members authorized by Managed Care Plans across the county. You will coordinate housing deposits, tenancy support, and link clients to HUD-funded resources while collaborating with CoC partners.

The role emphasizes housing stability, knowledge of HUD funding, and the ability to work with diverse populations. A California driver license and flexible approach are preferred to support seamless housing

Qualifications

• High School Diploma or GED and four (4) or more years of relevant experience required; or equivalent combination of education and experience.
• Knowledge of housing rights pertaining to homelessness, and low- and moderate-income housing.
• Knowledge of Coordinated Entry Systems (CES), local Continuum of Care system, local housing authorities, and ability to create partnerships with local landlords and property management companies.
• Training in Motivational Interviewing, Trauma Informed Care, Harm Reduction and related trainings.

Responsibilities

• Assist clients with housing needs and coordinate housing resources.
• Develop individualized housing plans and link to subsidies and affordable options.
• Collaborate with CoC, MCPs, and ECM providers to deliver housing services.
• Maintain documentation in HMIS and Salesforce per HUD requirements.
• Conduct weekly meetings with clients to monitor progress.
