Job summary

The Center for Urban Community Services (CUCS) seeks a Case Manager for The Lenninger Residence, a 92-unit permanent supportive housing community. You will provide person-centered case management to support clients in achieving housing stability, health, recovery, and self-sufficiency.

Key responsibilities include developing individualized plans, helping clients secure benefits and housing, crisis intervention, and onsite/field services with thorough documentation.

Qualifications

• Education/Experience: HS Diploma + 2 yrs (CM I) to Bachelor’s +4 yrs (CM IV).
• Strong communication, organization, and crisis management skills.
• Bilingual preferred; ability to travel across NYC.
• Valid driver’s license may be required.

Responsibilities

• Develop and update individualized service plans based on client strengths and goals.
• Assist clients with securing benefits, maintaining housing, and accessing healthcare/community resources.
• Provide crisis intervention, medication support, and assistance with activities of daily living.
• Deliver services onsite and in the field (home visits, accompanying clients to appointments).
• Maintain timely, accurate documentation and client records per regulatory standards.
