Responsibilities
About the Role:
The Center for Urban Community Services (CUCS) seeks a Case Manager for The Lenninger Residence, a 92-unit permanent supportive housing community. You will provide person-centered case management to support clients in achieving housing stability, health, recovery, and self-sufficiency.
Key Responsibilities:
Develop and update individualized service plans based on client strengths and goals.
Assist clients with securing benefits, maintaining housing, and accessing healthcare/community resources.
Provide crisis intervention, medication support, and assistance with activities of daily living.
Deliver services onsite and in the field (home visits, accompanying clients to appointments).
Maintain timely, accurate documentation and client records per regulatory standards.
Requirements:
Education/Experience: Ranging from HS Diploma + 2 yrs experience (CM I) to Bachelor’s + 4 yrs experience (CM IV). College credits may substitute for experience.
Skills: strong communication, organization, and crisis management skills; computer proficiency; bilingual preferred.
Physical/Field Work: Ability to travel throughout NYC, escort clients via public transit, and lift up to 30 lbs. Valid driver's license may be required.
