Job summary

Alameda Alliance for Health is seeking a Housing Community Supports Specialist to bridge homelessness, housing, and healthcare. You will work with internal teams and external partners to provide social service linkage and housing options for members, ensuring culturally and linguistically aligned support while maintaining compliance.

You will manage complex housing referrals, coordinate with housing programs, and support transitions from homelessness to community-based housing, leveraging harm

Qualifications

• Bachelor’s degree in counseling, social work, public administration, healthcare administration, or related field.
• LCSW/LMFT/LPCC or related licensure preferred.
• Three–Five years’ experience in Permanent Supportive Housing or related housing services with caseloads.
• Experience with data entry systems such as TruCare, HMIS, or CHR.

Responsibilities

• Process accurate and timely approvals/denials for Housing CS requests.
• Coordinate communications between CS providers/vendors, CBOs, ECM, and Alliance Case Management.
• Identify housing resources and referrals to help members access services.
• Maintain and track data and case progress; ensure timely case closures.
