Job summary

Alameda Alliance for Health seeks a Housing Community Supports Specialist to bridge homelessness, housing, and healthcare. You will collaborate with ECM, case management, and external partners to provide social service linkage and ensure referrals to community resources are aligned with members’ housing goals.

Responsibilities include processing approvals, coordinating with providers, and maintaining detailed service notes while upholding Harm Reduction and Housing First practices.

Qualifications

• Bachelor's degree in counseling, social work, public administration, healthcare administration, or a closely related field.
• LCSW, LMFT, LPCC or related licensure preferred.
• 3-5 years experience in Permanent Supportive Housing or related housing supports with data-entry experience (e.g., HMIS, CHR).
• Experience working with individuals with co-morbidity (mental illness, substance use, trauma).
• Deep knowledge of Alameda County community resources for extremely low-income/unhoused individuals.
• Knowledge of Harm Reduction, Housing First, Trauma-Informed Care practices.

Responsibilities

• Process housing CS approvals/denials via the enterprise care management platform.
• Notify members/providers of authorization outcomes.
• Coordinate care with CS providers, ECM, County partners, and CBOs.
• Use housing CS guidance for screening, approving, finalizing requests.
• Perform data entry and track processes/cases.
• Interpret Housing CS services and advise Alliance staff on eligibility issues.
• Prioritize approvals for Housing Program Manager and vendors.
• Identify community resources to support independent housing options.
• Assist with linking members to health and social services.
• Collaborate with local departments and external stakeholders on housing options.
• Coordinate referral/resource linkage with contracted vendors and partners.
• Support integrated services team model and client-centered collaboration.
• Review Housing Needs Assessments to align supports in least restrictive environments.
• Manage large caseloads and monitor case progress.
