Job summary

City Net seeks Case Managers to provide housing-focused case management and supportive services to individuals and families experiencing homelessness at a non-congregate interim housing site. You will meet clients where they are, helping them set and reach long-term housing goals and connect them to medical, mental health, counseling, and housing resources.

You will work with partners across nonprofits, government, and healthcare to coordinate care and move clients toward permanent housing as

Qualifications

• Must be eligible to drive a City Net vehicle and meet insurance requirements.
• Demonstrated empathy, self-motivation, flexibility, and adaptability.
• Experience building relationships with diverse stakeholders.
• Strong data collection and reporting capabilities.
• Professional boundaries and excellent communication skills.
• Ability to work independently and in a team.

Responsibilities

• Meet clients for housing-focused case management and assist in developing long-term housing plans.
• Help clients find and access housing programs and complete housing applications.
• Screen clients for eligibility and provide referrals to appropriate services.
• Advocate for clients with partner agencies and accompany them to appointments when needed.
• Collect data accurately and timely; enter HMIS and CES records and follow protocols.
• Meet or exceed productivity and outcome goals and comply with contracts and City Net procedures.
