Job summary

EMPACT‑SPC/La Frontera Arizona is seeking a Peer Support Navigator (Supportive Housing) to provide case management, emotional support, and advocacy for chronically homeless adults within Mercy Maricopa referrals. You will map steps to access housing and services, collaborate with community partners, and document progress in electronic records.

The role requires a High School Diploma, valid AZ driver license, and fingerprint clearance.

Qualifications

• Strong interpersonal and communication skills for client engagement.
• Ability to interpret and support recovery plans.
• Commitment to trauma-informed and recovery-oriented practice.
• Ability to collaborate with community resources and service providers.

Responsibilities

• Meet with clients in the community and map next steps for access to services.
• Develop and maintain networks with shelters, housing resources, and RBHA.
• Document progress in HMIS/HOME Link and client records.
• Provide peer support, teaching self-management techniques and reassurance.
• Complete required training in risk assessment and housing-first practices.
