Job summary

La Frontera EMPACT - Suicide Prevention Center is seeking a CCHP Peer Support Navigator to provide case management, emotional support, and advocacy for chronically homeless adults within Mercy House housing. You will map steps, access community resources, and maintain detailed records to promote engagement with outpatient services and natural supports.

The role emphasizes crisis intervention, self-directed recovery support, and collaboration with a broad network of social/behavioral health

Qualifications

High School Diploma or equivalent required.
Peer Certification required.
Must identify as a recipient of mental health services (in Maricopa County) or have assisted family member in their mental health or substance abuse recovery process.

Responsibilities

Meets with service recipients in the community to map out next steps for access to providers or community-based services.
Develops and collaborates with a network of Community Based and RBHA resources to access housing, food, clothing, and services.
Maintains accurate documentation in progress notes and EMPACT’s HMIS/HOME Link systems.
Provides peer support and helps participants develop self-directed recovery plans and coping techniques.
Coordinating housing with community shelters, apartments, and housing facilities.
