Job summary

La Frontera Arizona is seeking a Peer Support Navigator to work with Housing Programs in Tempe, AZ. The role involves meeting clients in the community, coordinating with shelters and RBHA resources, and supporting recovery plans within EMPACT-SPC services.

You will document case notes, collaborate with treatment teams, and assist with transportation and access to services. A GED/high school diploma and AZ driver’s license are required, with HIPAA-adherent record-keeping expected.

Qualifications

• High School Diploma or equivalent required.
• Must identify as a recipient of mental health services (in Maricopa County) or have assisted a family member in recovery.
• Effective interpersonal skills to build professional relationships.
• Effective communication skills with non-judgmental approach and active listening.
• Thorough understanding of family support at every stage of recovery.
• Ability to interpret and implement complex treatment plans using community services.

Responsibilities

• Meets with service recipients in the community and helps identify next steps.
• Maps an extensive network of community-based resources and RBHA services.
• Maintains accurate documentation in progress notes and HMIS/Home Link.
• Provides peer support and helps clients develop self-directed recovery plans.
• Participates in agency fund raising activities as requested by management.
