- Provide outreach, engagement, and case management per ISP.
- Assess, plan, and implement services; document progress toward recovery.
- Coordinate care across behavioral health, medical, dental, and community resources.
- Maintain clinical records and collaborate with families and providers.
- Assist in monitoring care levels and participate in quality activities.
