Job summary
La Frontera EMPACT - Suicide Prevention Center is seeking a SMI Case Management Navigator to provide outreach, engagement, and case management across behavioral health services in the Phoenix metro area. The role focuses on assessments, service plans, and coordinating care with families, providers and community resources to support recovery.
Ideal candidates bring background in community behavioral health, strong communication skills, and ability to work with seriously mentally ill and homeless
Qualifications
- Bachelor’s degree or equivalent in a related field; ADHS BHT requirements may apply.
- Experience in community behavioral health case management and outreach to seriously mentally ill or homeless populations.
Responsibilities
- Provide outreach to engage consumers not currently in services and coordinate with families and providers.
- Complete assessments and service plans; document progress toward recovery.
- Maintain and monitor behavioral health services and level of care.
- Coordinate care across multiple agencies and communicate with involved parties.
