Job summary

The Coordinating Center is seeking a Homeless Support Services Coordinator to support the Housing Initiative Program in Montgomery County. The role focuses on delivering person-centered services, coordinating with CES and HIP resources, and helping formerly homeless individuals achieve permanent housing stability through Housing First practices.

The position is full-time and teleworking, with required home and community visits across Montgomery County, MD.

Qualifications

• Bachelor’s degree in a human services related field is required.
• Two years of case management experience is required.
• Ability to travel throughout Montgomery County and conduct home and community visits is required.

Responsibilities

• Coordinate Housing First-based services to maintain housing stability for formerly homeless individuals.
• Provide person-centered services and documentation within HIP contract requirements.
• Engage with CES, HIP Housing Locator and Community Health Nurse as applicable.
• Maintain HMIS and other databases with accurate, timely records.
• Conduct outreach to program recipients and collaborate with community agencies to support residential stability.
