Job summary

Shelter House, Inc in Fairfax County, VA is seeking an Entry Community Housing Case Manager to support 15-20 families in Prevention and Rapid Rehousing programs, conducting needs assessments and creating housing stability plans.

You will work closely with landlords, service providers, and county partners to link households to resources, manage documentation in HMIS, and help clients achieve sustained housing.

Qualifications

• Bachelor's degree in human services or commensurate experience.
• 1+ years of human services or direct services experience.
• Ability to prioritize competing priorities and make sound judgments.
• Ability to advocate, assist in problem-solving, and achieve results for families.
• Strong written and oral communication skills.
• Strong organizational and time management skills.
• Valid Driver's License and reliable transportation.
• Flexibility to work outside of normal business hours when needed.

Responsibilities

• Conduct comprehensive assessments and screening of housing history, barriers, health, needs, and goals.
• Assist clients in achieving long-term housing stability by lease compliance and housing plans.
• Meet regularly with households to provide tailored case management and referrals.
• Provide housing navigation support and landlord engagement assistance.
• Assist with financial documentation and eviction process when needed.
• Prepare and document rental assistance packages and recertifications.
• Mediate conflicts among households, roommates, or landlords.
• Support budgeting and money management for households.
• Maintain relationships with landlords and community partners.
• Maintain contact with households relocating or changing numbers to ensure engagement.
• Document all contacts within 48–72 hours in HMIS or equivalent.
• Participate in collaborative meetings with Fairfax County and partner organizations.
• Help clients overcome barriers like arrears, credit history, and understanding tenant rights.
• Develop and maintain a comprehensive directory of local/state service providers.
• Maintain thorough case documentation per agency standards.
• Collaborate as part of Shelter House team.
