Job summary

Continuum of Care in New Haven, CT, seeks a Case Manager I for the New Haven Halfway House program. The role focuses on supporting clients toward independence through comprehensive case management, screenings, and person-centered care planning, with a team-based approach and strong emphasis on documentation.

Responsibilities include developing plans, coordinating services (entitlements, housing, transportation), and facilitating psychoeducation groups as needed.

Qualifications

• Bachelor's degree or high school diploma with relevant experience in human services.
• Experience in residential or community-based case management and team environments.
• Ability to maintain documentation and coordinate services across providers.

Responsibilities

• Establish rapport with clients and maintain professional boundaries.
• Develop and monitor person-centered plans with clients.
• Coordinate referrals, entitlements, money management, housing access, and transportation.
• Conduct regular progress reviews and document outcomes per contract requirements.
