Continuum of Care is seeking a Case Manager I for a 1st shift, full-time role in New Haven, CT. You will assess clients, develop person-centered plans, and connect them with community resources to promote independence and well-being.

The position emphasizes a team approach, documentation, and client-centered support in housing, transportation, and daily living skills. Qualified candidates have a Bachelor’s degree preferred with 1 year of relevant experience (2 years with a high school diploma).

Qualifications

• Bachelor’s degree preferred with 1 year of experience; high school diploma accepted with 2 years of relevant experience.
• Experience with individuals with persistent mental illness or addiction preferred.
• Valid driver's license and reliable vehicle required for community-based settings.
• CPR/First Aid certification offered as part of orientation and to be maintained.

Responsibilities

• Establish rapport and build positive client relationships while maintaining professional boundaries.
• Develop, implement, and monitor person-centered care plans with clients from intake to discharge.
• Coordinate referrals, entitlements, money management, and access to treatment and housing.
• Assist clients in overcoming barriers and linking to appropriate services.
• Meet with clients regularly to review progress and appointments; provide transportation as needed.
• Complete service entry documentation and maintain a clean, safe living environment.
