Job summary

Pine Street Inn is hiring a Permanent Housing Case Manager to deliver intensive case management to tenants within a Housing First framework. The role focuses on promoting housing stability and coordinating access to benefits, health care, education, employment, and legal aid.

You will support tenants with daily living skills and create individualized service plans while documenting progress. The position emphasizes Harm Reduction, trauma-informed approaches, and collaboration with a dedicated

Qualifications

• High School Diploma or Equivalent.
• Strong written communication skills.
• Computer Proficiency in Microsoft Office Products.
• Bi-lingual – Spanish/English (preferred)

Responsibilities

• Provide intensive case management to homeless/chronically homeless individuals.
• Assist tenants with benefits, health care, education, employment opportunities, and legal aid.
• Develop individual service plans and maintain necessary documentation.
• Collaborate with Permanent Housing staff on program-wide efforts to stabilize tenants.
• Utilize Harm Reduction and Motivational Interviewing techniques in interactions with participants.
