Job summary

Miriam’s Kitchen is seeking a Case Manager for the E Street Bridge Housing Program in Washington, DC. The role focuses on trauma-informed, client-centered services to help residents move to stable housing, with a caseload of about 25 individuals and daily on-site duties.

The position is full-time, on-site, with a Monday–Friday schedule and occasional late-day rotations. Strong teamwork, documentation, and knowledge of housing supports are essential to support residents’ goals.

Qualifications

• 1+ year of experience in homelessness, mental health, substance use, HIV/AIDS, or case management for vulnerable populations.
• Associate degree or 2 years of college; exceptions for DHS Peer Case Management Institute.
• Spanish proficiency preferred.
• Ability to lift up to 25 pounds and operate basic computer databases.

Responsibilities

• Deliver client-centered, trauma-informed case management to help residents exit to permanent housing.
• Maintain a caseload of ~25 residents with consistent, individualized support.
• Conduct intake and needs assessments using standardized tools.
• Develop and implement individualized case plans covering housing, health, and supports.
• Coordinate referrals and warm handoffs to internal and external providers.
• Complete timely documentation in required databases and formats.
• Attend multidisciplinary meetings and trainings; participate in program development.
