Miriam’s Kitchen is hiring a PSH Case Manager to provide community-based support to a primary caseload of up to 18 residents formerly chronically homeless. You will help move individuals from homelessness to housing, coordinating housing identification and ongoing support to sustain stable housing and self-sufficiency.

Responsibilities include outreach, crisis support, advocacy, and collaboration with landlords and service providers, with work conducted in residents’ homes, Miriam’s Kitchen

Qualifications

• Bachelor’s degree in social work, psychology, sociology, counseling, or a related social service or science discipline, AND one year of experience providing case management services; or a master’s degree in a related field.
• Enthusiastic about the Housing First approach.
• Proficiency in Spanish is preferred.
• Strong written and verbal communication skills.
• Valid driver’s license and personal vehicle.
• Ability to pass background checks as required by government contract.
• Commitment to ending chronic homelessness in DC.
• Understanding of the structural and systemic causes of homelessness.

Responsabilités

• Develop trusting relationships with PSH residents; collaborate on goals and service plans for housing, mental health, substance abuse, health, income, employment and more.
• Provide outreach, case management, crisis support, advocacy and other services in the community and residents' homes.
• Meet or exceed monthly client contact requirements (two contacts per month, one home visit).
• Support residents’ wellness and independence.
• Monitor residents’ medical and mental health needs and facilitate referrals.
• Act as liaison with landlords and community stakeholders to promote resident success.
• Collaborate with service providers to ensure high-quality services.
• Maintain written and electronic records and ensure confidentiality.
