Job summary

Friendship Place is seeking a Case Manager for the Neighbors First Program, delivering permanent supportive housing services with a Housing First approach in the District of Columbia. The role emphasizes empowering participants through holistic, client-driven planning and wraparound resources.

You will conduct face-to-face outreach, assess needs, create service plans, and coordinate with landlords and service providers to support housing stability and wellness.

Qualifications

• Bachelor’s or higher in a related field preferred but not required with relevant experience.
• Certification in First Aid/CPR/AED may be required or provided.
• Bilingual Spanish/English proficiency strongly preferred.
• Experience with homeless services and wraparound care is desirable.

Responsibilities

• Provide supportive services to individuals experiencing homelessness with complex needs.
• Spend ~80% of time in the field delivering face-to-face contact.
• Develop, implement, and revise individual service plans.
• Coordinate wraparound services and housing resources for participants.
• Monitor medical/psychiatric needs and medication management.
• Maintain electronic records in the Housing the Homeless database.

High School diploma or GED + 6 years human service experience

Bachelor’s degree + 2 years human service experience
