Job summary

Miriam’s Kitchen in Washington, DC, is seeking a PSH Case Manager to provide community-based support to a caseload of up to 18 residents who were formerly chronically homeless. You will work with residents to identify housing goals, coordinate services, and facilitate connections to landlords and service providers, helping residents remain stably housed and achieve self-sufficiency.

• Bachelor’s or Master’s degree in social work, psychology, sociology, counseling, or related field; 1 year of case management experience preferred.
• Enthusiasm for Housing First approach.
• Proficiency in Spanish preferred; ability to work with diverse population.

Responsibilities

• Engage PSH residents to develop goals and service plans for housing, health, income, and other areas.
• Provide outreach, case management, crisis support, advocacy, and services primarily in the community and residents’ homes.
• Meet monthly contact requirements including home visits to support wellness and independence.
• Maintain confidential program records and documentation in accordance with standards.
