Job summary

Step Up Nashville is seeking a Housing Case Manager to deliver Housing First based services to adults experiencing chronic homelessness in the Nashville area. The role focuses on engagement, assessment, service planning, and support for securing and maintaining permanent housing.

You will document care using BIRP notes, coordinate with community resources, and participate in team activities to promote housing stability in a traumainformed, recovery-oriented framework.

Qualifications

• Minimum of 2 years related housing/case management experience with chronically homeless adults and co-occurring disabilities.
• Knowledge of housing vouchers and providing field-based services.
• Experience documenting services using the BIRP note format.

Responsibilities

• Provide intensive Housing First case management and linkage to clinical and community resources.
• Conduct regular home visits to review progress and build skills for housing stability.
• Coordinate care and maintain relationships with community resources for successful transition to independent living.
• Monitor for warning signs of crisis and de-escalate as part of the team.
• Document services using BIRP notes and HMIS in a timely manner.
