Job summary

PHC in Des Moines, IA is seeking an Economic Development Case Manager to assist clients experiencing homelessness with employment and housing goals. You’ll support intake, document needs, coordinate with medical, dental, mental health, and external agencies, and help with transportation and benefits enrollment.

Responsibilities

Completes intakes and ongoing assessment with clients to identify needs, develop goals, and access resources and services.
Monitors and documents client contacts, needs, goals, progress, and significant occurrences, according to grant guidelines and PHC policies and procedures.
Facilitates coordination with other PHC services, including medical, dental, mental health, and substance use, as well as referrals for services with external agencies.
Advocates for clients with community agencies, schools, significant others and other appropriate agencies.
Ensures clients have access to transportation to necessary appointments as appropriate, assisting with client transportation in accordance with PHC transportation policy.
Communicates with other agencies on behalf of clients using professional discretion with client information, ensuring that releases of information are obtained.
Conducts outreach activities at community agencies, community gatherings, neighborhoods, shelters, and other appropriate venues as requested.

Primary Health Care (PHC) was founded in 1981 by Dr. Bery Engebretsen in Des Moines, IA. Our mission has remained unchanged since that time, to provide healthcare and supportive services to all, regardless of insurance, immigration status, or ability to pay.

Based on the needs of the communities we serve, PHC offers a spectrum of medical and dental services including family practice, behavioral health, HIV care and services, and pharmacy. PHC’s Homeless Support Services is the entry point for serving people experiencing homelessness in Polk County. Enabling services are available to help patients with benefits enrollment, case management, transportation, translation, and patient education. We currently have locations in Ames, Des Moines, & Marshalltown.

As an Economic Development Case Manager, you will help clients close the gap between income and housing. The EDCM will serve clients who are currently experiencing homelessness in Polk County and who are seeking employment or seeking income to supplement their SSI. EDCM will assist clients with obtaining employment including gaining vital documents, writing resumes, and helping with transportation to interviews. The EDCM will assist clients to obtain housing in the community and work towards client self identified goals. This position is time limited and based upon grant funding.

What You Will Do

Advocates for clients with community agencies, schools, significant others and other appropriate agencies.
Ensures clients have access to transportation to necessary appointments as appropriate, assisting with client transportation in accordance with PHC transportation policy.
Communicates with other agencies on behalf of clients using professional discretion with client information, ensuring that appropriate releases of information are obtained.
Conducts outreach activities at community agencies, community gatherings, neighborhoods, shelters, and other appropriate venues as requested. Serves as a liaison between PHC and community resources.

Preferred:

Six months of work or volunteer experience in human services related activities. (Will consider experience acquired while obtaining the degree.)
Work experience with vulnerable populations, including people experiencing homelessness.
Demonstrated competency using HMIS as applicable to department.
Working knowledge of community resources related to homelessness in Des Moines.
Knowledge of program terminology.
Ability to effectively use HMIS system to document all client contact.
Demonstrate language proficiency in Spanish.

Your experience and skills determine your base pay. The hiring range for this position is typically $17.00 - $21.25per hour. Candidates with extensive work experience related to this position may be considered for additional compensation up to the pay grade maximum of $25.50 per hour.
