Terros Health in Tempe, AZ is seeking a Health and Housing Specialist to join our interdisciplinary clinical team providing community-based services to adults with substance use disorders, serious mental illness, or dual diagnoses.

The role delivers direct housing support, assessments, tenancy coaching, and coordination with medical and behavioral health providers while offering transportation and client advocacy.

Qualifications

• High school diploma or GED with 1.5 years of behavioral health experience or an associate’s degree in medical/behavioral health or a bachelor’s in a non-behavioral field with 1 year BH experience or a bachelor’s or higher in behavioral health.
• Experience with behavioral health services and ability to engage clients in community-based housing services.
• Applicant must have strong communication, independence, and reliability to support housing prospects.

Responsibilities

• Provide direct face-to-face community-based housing services to assist clients to obtain and maintain housing.
• Conduct screenings, intakes, and assessments to determine needs and treatment goals.
• Coordinate with family, medical and behavioral health providers, and community resources.
• Assist clients with connection to physical health services and primary care follow-ups.
• Provide transportation as needed and maintain ongoing collaboration with the clinical team.
• Deliver high-quality customer service to clients, providers, and stakeholders.
