Program Description:
The Reentry Housing Program is a scatter site transitional housing program that provides supportive housing services to approximately 90 men/women parolees being released from New York State Correctional facilities and have been diagnosed with serious mental illness and/or substance use disorders. The multidisciplinary team consists of Intensive Case Managers, Supportive Case Managers, Housing Counselors, psychiatric providers, and occupational therapists who work collaboratively to promote housing stability, improve client well-being, and support successful community reintegration. Additionally, the program offers a comprehensive array of supportive services designed to enhance clients’ stability, independence, and long‑term recovery.
Overall Responsibilities:
Under the general supervision of the Program Director, the Housing Coordinator is responsible for managing approximately 45 scattered‑site apartments located throughout the Bronx and Manhattan. Responsibilities include lease administration, apartment preparation and furnishing, coordinating maintenance and repairs, responding to clients’ housing needs, ensuring apartments remain safe and habitable, identifying new housing opportunities, preparing reports for the Program Director, and performing other duties as assigned.
