Job summary

Project Renewal is a leading non-profit in New York City seeking a Housing Case Manager to support homeless adults in our housing programs across the Bronx, Brooklyn and Manhattan. Salary ranges from $38,000 to $46,000 per year, with opportunities at multiple housing sites including Bedford Greenhouse I & II, The Villa, and several scattered sites.

The role requires a Bachelor’s in Social Work or related field, 1–2 years of relevant experience, strong written and verbal communication, and

Qualifications

• Bachelor’s degree required.
• 1–2 years of experience with homeless mentally ill adults or similar population preferred.
• Excellent oral, writing and listening skills; proficiency with computers.

Responsibilities

• Provides intake, psychosocial and substance abuse assessments and develops treatment plans.
• Manages individual Service Plans with short- and long-term goals.
• Conducts home visits and coordinates medical, psychiatric and substance abuse services.
• Monitors medication compliance as necessary.
• Maintains case notes in AWARDS/CARES system.
• Conducts individual and group supportive counseling sessions.
• Organizes tenants education groups on money management, employment, etc.
• Links clients to community services and resources.
• Participates in weekly clinical meetings and updates progress spreadsheets.
• Performs other duties as assigned by supervisors.

Essential Duties and Responsibilities:

The essential duties of the Housing Case Manager include but are not limited to the following activities:

• Provides intake, psychosocial and substance abuse assessments to and develop an appropriate treatment plan for each client in the assigned caseload.
• Provides case management services to assist in the creation of and adherence to individual Service Plans, identifying short-term and long-term goals and objectives to be achieved.
• Conducts home visits, coordinates medical, psychiatric and substance abuse services
• Monitors medication compliance as necessary
• Maintains written information or case notes regarding all contact and intervention with the clients in AWARDS/CARES system
• Conducts individual and group supportive counseling sessions
• Organizes and educates tenants through groups and other activities presenting issues of interest to the clients such as money management, employment opportunities, etc.
• Links clients to community services
• Participates in weekly clinical meetings to discuss high risk clients; updates spreadsheets related to clients’ progress
• Performs other duties as assigned by supervisory staff.

Qualifications:

• Bachelor’s Degree in Social Work or related field of experience required
• 1-2 years of experience working with homeless mentally ill adults or other special needs population preferred
• Excellent oral, writing and listening skills
• Proficiency in computer required.
• Bilingual or multilingual a plus
