Job summary

Project Renewal's New Providence Women’s Shelter in New York City is seeking a Case Manager to complete intake assessments, coordinate services, and support clients in pursuing Independent Living Plans.

The role requires at least two years' experience with homeless or similarly challenged populations; you will document in CARES, assist with housing, benefits, and employment opportunities, and collaborate with a multidisciplinary team.

Qualifications

• Minimum of two (2) years of experience working with homeless, mentally ill and chemically addicted individuals or other special needs populations.

Responsibilities

• Conducts intake and assessments; makes referrals and completes psycho-social assessments.
• Provides case management and assists clients with Independent Living Plans and objectives.
• Inputs client information into CARES and maintains charts.
• Collaborates with clients and staff to obtain entitlements, housing, and services.
• Maintains accurate client documentation per guidelines.
• Provides guidance on personal goals, housing, sobriety, medications, and employment.
• Maintains liaison with community organizations.
• Provides escort services to appointments as needed.
• Attends program meetings and contributes to team discussions.
• Performs other duties as assigned by leadership.

Salary: $41,000 - $45,000

Essential Duties and Responsibilities:

The essential duties of the Case Manager include but are not limited to the following activities:

• Conducts intake and assessments, makes necessary referrals for services and completes/updates psycho-social assessments of clients.
• Provides case management services and assists clients in the creation of and adherence to their Independent Living Plans by identifying short and long-term objectives of which can be achieved.
• Inputs all client information and updates in a timely fashion into the CARES system as well as the client’s physical chart.
• Works collaboratively with clients and other staff to identify and obtain financial entitlements, housing and other concrete services
• Maintains accurate and up to date written client documentation as required and in accordance with City/State oversight agency guidelines
• Provides guidance for clients through individualized session on issues such as achieving personal goals, finding permanent or alternative housing, maintaining sobriety, complying with medication protocols and pursuing employment opportunities.
• Maintains liaison with representatives from community organizations.
• Provides escort services to clients’ appointments as needed.
• Attends and participates in the program meetings.
• Performs other duties as assigned by the Team Leader, Clinical Director or Program Director.

Physical Activities:

While performing the duties of this position, the Case Manager must walk between all floors of the facility, climbing up and down staircases to meet with clients in the event the elevator is out of order. The CM must stand talking to clients and staff, sit for extended periods of time at the computer, and reach or squat to retrieve file records.

Qualifications:

• Minimum of two (2) years of experience working with homeless, mentally ill and chemically addicted individuals or other special needs populations.

Preferred Skills:

• Team Building: Ability to work cooperatively with peers and other staff in order to serve the needs of the clients as effectively and efficiently as possible.
• Organizational: An ability to work independently and to adjust one’s schedule to accommodate the needs of the clients.
• Communication: Excellent oral, writing, and listening skills.
• Interpersonal: An ability to interface with clients as well as all levels of staff.
• Computer: Knowledge of Case Management software (CARES). Proficiency in Microsoft Office Suite, especially Outlook, Word, and Excel.
• Language: Bilingual or multilingual a plus.
