Job summary

Project Renewal Inc. in New York City seeks a Harm Reduction Case Manager for the In Homes Now program to support 116 residents transitioning to permanent housing.

The program uses a Harm Reduction/Housing First approach, with housing provided across the Bronx, Brooklyn, and Manhattan. The role requires managing a caseload of 16–18, meeting clients at least twice a month in their homes, coordinating referrals, and maintaining accurate clinical and statistical records.

Qualifications

• Ability to complete assessments and service plans.
• Experience working with individuals who are homeless and/or affected by substance abuse.
• Strong time management and writing skills.

Responsibilities

• Manage a caseload of 16–18 individuals.
• Meet clients at least twice monthly in home.
• Refer clients to medical/dental and substance abuse services.
• Maintain accurate clinical and statistical documentation.
• Complete service plans every 6 months.
• Complete psychosocial assessments annually.
• Escort clients to appointments.
• Attend staff meetings and in-service trainings; participate in case consultations.
