Job summary

Henry Street Settlement seeks a dedicated professional for the Transitional & Supportive Housing program on Third Street Women’s Residence. This on-site role involves coordinating case management, counseling, and housing placement services for unhoused women with behavioral health needs.

Responsibilities include ILP development, safety assessments, housing applications, and collaboration with DHS. A BS or BSW and 3+ years in related work are preferred.

Qualifications

• Bachelor's degree required; BSW preferred.
• Minimum of three years' experience with homeless or mentally ill populations.
• Experience with DHS CARES system strongly preferred.
• Proven ability to provide comprehensive case management.

Responsibilities

• Coordinate and deliver comprehensive case management for residents.
• Provide individual counseling and crisis intervention.
• Conduct floor walks to assess safety and improve daily living skills.
• Clearly communicate shelter rules, expectations, and ILP responsibilities.
• Build and sustain relationships with housing providers, real estate brokers, and landlords to facilitate housing placements.
• Assist residents with housing applications and subsidy paperwork.
