Job summary

Henry Street Settlement seeks a dedicated professional for the Transitional & Supportive Housing program on Third Street Women’s Residence. This on-site role involves coordinating case management, counseling, and housing placement services for unhoused women with behavioral health needs.

Qualifications

  • Proven ability to provide comprehensive case management.

Overview

Job Overview:
Division/Department: Transitional & Supportive Housing – Third Street Women’s Residence
Work Schedule: 35 hours per week, Regular, Full Time: Monday, Tuesday, Wednesday, Friday 9 am -5 pm, Late Night: Thursday 1 pm -9pm.
Exemption Status: Non-Exempt

Building on more than 125 years of innovation and community impact, Henry Street offers over 50 programs and is one of New York City’s largest and most comprehensive social service agencies.

The Transitional & Supportive Housing program provides temporary emergency housing for single adult women who are unhoused and have behavioral health issues. Residents are referred by the New York City Department of Homeless Services (DHS). The program offers a safe, supportive environment designed to help residents achieve stability and transition to independent living.

Responsibilities
  • Coordinate and deliver comprehensive case management services for residents, including the development and implementation of Independent Living Plans (ILPs)
  • Provide individual counseling, crisis intervention, and ongoing support to assist residents in achieving their independent living and housing Goals
  • Conduct floor walks and meetings to assess for safety and improve the residents’ independent daily living skills
  • Clearly communicate shelter rules, expectations, and ILP responsibilities to residents
  • Accompany residents to and from appointments, such as public assistance, apartment viewings, interviews, lease signings, and furniture shopping, as needed
  • Ability to travel to all five boroughs using public transportation
  • Need to demonstrate flexibility with work hours to meet client and programmatic needs
  • Maintaining current knowledge of housing resources and social services supports relevant to residents’ needs
  • Build and sustain relationships with supportive housing providers, real estate brokers, and landlords to facilitate housing placements
  • Maintain an active roster of housing providers and update contact lists regularly
  • Assist residents with housing applications and subsidy paperwork
  • Facilitate workshops and rehabilitative groups to prepare residents for permanent housing opportunities
  • Achieve a minimum of four (4) permanent housing placements per quarter
  • Participate in rotating backup coverage for the facility as assigned
  • Open/Close and Maintain Client Charts in accordance with the DHS/OTDA policies and procedures
  • Perform other related duties as required to support the program’s goals and overall success
Essential Physical Job Functions
  • Ability to sit and stand for extended periods
  • Ability to climb multiple flights of stairs throughout the day
Requirements
  • Bachelor’s degree required; Bachelor of Social Work (BSW) preferred
  • Minimum of three (3) years of experience working with homeless and/or mentally ill populations
  • Experience with the DHS CARES system strongly preferred
  • Demonstrated ability to provide comprehensive case management services
  • Strong organizational, problem-solving, and judgment skills
  • Excellent verbal and written communication abilities
  • Ability to remain calm and professional in challenging situations
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook)
  • Knowledge and appreciation of team-based approaches to service delivery
