Job summary

Volunteers of America Upstate New York is seeking a Case Manager to provide housing case management for a caseload, coordinating with HUD and local partners to ensure access to services and stable housing.

You will conduct eligibility assessments, schedule intake, and perform home visits, documenting progress and updating plans as needed. Candidates should have an associate degree in human services and a valid NYS driver’s license.

Qualifications

• Associates degree in Human Services or related field preferred.
• Bachelor’s degree and at least one year experience preferred.
• Clean NYS Driver’s License and strong organizational skills required.

Responsibilities

• Provide housing case management for a caseload per VOA policies and HUD standards.
• Collaborate with Rochester Housing Authority to deliver HUD services.
• Receive referrals within 48 hours and document in SCN and Find Help.
• Schedule intake, verify eligibility, and provide necessary documentation to RHA.
• Conduct regular in-person sessions and home visits at least monthly.
• Maintain records per VOA policies and HMIS and prepare reports.
