Job summary

Mercy Housing & Shelter Corp. seeks a Case Manager to support the Haven Program for ten young adults aged 18-25. You will ensure high-quality services, treat residents with respect and compassion, and help each client achieve full, productive integration into the community.

The role requires knowledge of evidence-based practices, crisis intervention, and strong documentation skills. Flexibility for nights and weekends is essential as part of the schedule.

Qualifications

  • Master’s Degree in Social Work or related field preferred.
  • 3–5 years of experience in social work, rehabilitation therapy, or behavioral science.
  • Ability to work with multicultural clientele.

Responsibilities

  • Knowledgeable of evidenced-based practices and interventions.
  • Provide individual, group and/or family supportive treatment under supervision.
  • Provide crisis intervention services.
  • Participate in nursing, clinical or rehabilitative interventions to meet client needs.
  • Document client contact, treatment, and progress.
  • Advocate for services or resources meeting client needs and preferences.
  • Update rehabilitation plans with progress of client.
  • Observe and record progress within the rehabilitation plan.
  • Teach independent living skills (e.g., budgeting, shopping, etc.).
  • Coordinate with community agencies and outpatient appointments.

Monday – Friday, 8:30AM – 4:30PM; flexibility for nights and weekends are required.

Job Summary:

The Case Manager will work to support the Mission of Mercy Housing and Shelter Corp. and the Haven Program. The YAS Program at Mercy Housing and Shelter oversees ten young adults ages 18-25. The Case Manager will work to ensure the highest quality of service within the mission of the organization and will ensure program residents are treated with respect and compassion. The Case Manager holds primary responsibility for working towards the goal of each client’s full and productive integration with the community.

Duties & Responsibilities:
  • Knowledgeable of evidenced based practices both principles and techniques in all aspects of care (e.g., Integrated Dual Diagnosis, Person Centered Planning, etc.)
  • Able to provide individual, group and/or family supportive treatment under appropriate supervision.
  • Provide crisis intervention services.
  • The ability to participate in assigned nursing, clinical and/or rehabilitative interventions to meet client needs and identify support networks.
  • Document client contact, treatment, and progress.
  • Demonstrate the ability to advocate for services or resources that meet client needs and preferences.
  • Responsible for identifying client strengths and updates and modifies goals accordingly in the treatment plans.
  • Able to investigate alternative placement for clients.
  • Demonstrate the knowledge to address age, culture and gender specific characteristics when administering prescribed nursing interventions.
  • Updates rehabilitation plans with person in recovery at initial meeting and every 3 months or as needed.
  • Investigates and reports any unusual incidents; assures confidentiality of oral and written reports; coordinates support services such as dietary, maintenance, etc.
  • Participate in admission and discharge planning activities.
  • Knowledgeable in the practice of person-centered recovery principles.
  • Will observe and record progress within the rehabilitation plan.
  • Has the ability to teach independent living skills (i.e., cleaning, budgeting, shopping, safety, etc.
  • Knowledge of the impact of childhood trauma and other significant events in client’s development and functioning.
  • Participates as a liaison to community agencies; makes arrangements for outpatient appointments.
  • Foster young adult’s transition into adulthood by providing services that will enable them to function independently.
  • Ability to provide services which display attention to patient respect and dignity.
  • Perform any other task that will assist the Organization in carrying out its mission, as assigned.
Supervisory Duties & Responsibilities:
  • This position does not have any supervisory responsibilities. This position may be required to demonstrate the ability to instruct staff in crisis intervention techniques.

Education and Experience:

  • Master’s Degree in Social Work or related field preferred; plus, three to five years of experience in social work, rehabilitation therapy, or behavioral science.
  • Ability to work with multicultural clientele.
  • Clinical background with young adult experience preferred.
  • Knowledge of case management principles, practices, and procedures.
  • Considerable knowledge and experience in the dynamics of individual and group behavior.
  • Computer proficiency (i.e., databases, Microsoft Word, Excel), required.

Certificates and Licenses

  • Valid driver’s license, clean driving record and proof of current automobile insurance, and reliable transportation required.
  • $22-$24 per hour, based on experience.
