Job summary

The Fortune Society Inc. is seeking a Case Manager for Scattered Site Housing to provide assessments, plan development, and ongoing supportive services for justice-involved clients with histories of mental illness, substance use, and homelessness.

You will document services, conduct crisis interventions, and assist with benefits applications from Harlem, NY in a full-time, on-site role. Responsibilities include coordinating with landlords and benefits agencies, maintaining client records, and

Qualifications

• Associate’s Degree in Human Services or five years of experience with special needs population.
• Candidate must be able to oversee a caseload of 15 clients.
• Proficient in Microsoft Excel, Word, and Outlook with a working knowledge of PowerPoint.
• Excellent written and verbal communication skills.
• Excellent interpersonal and cross-cultural communication abilities.

Responsibilities

• Perform initial assessments and create service plans and Outcome Stars.
• Document services and outreach efforts and maintain records.
• Conduct crisis intervention as needed and assist with benefits applications.
• Liaise with benefits administrations and property management to resolve issues.
• Provide ongoing counseling and supportive services to achieve client goals.
• Travel across the five boroughs as needed to assist participants.
• Perform other duties as assigned.
