Job summary

The Fortune Society, Inc. is seeking a Case Manager (JISH) to perform initial assessments, provide ongoing supportive services in the field and office, and maintain documentation for justice-involved clients with histories of mental illness, substance use and homelessness.

The role requires travel across the five boroughs via mass transit, strong organizational skills, and the ability to coordinate with housing providers, benefits agencies, and program staff.

Qualifications

• Associates Degree in Human Services or five years of experience with special needs; HS diploma and eight years preferred.
• Ability to oversee a caseload of 15 clients with collaborative assessment and service planning.
• Proficiency in Microsoft Office applications including Excel, Word, Outlook and PowerPoint.
• Excellent written and verbal communication; interaction with diverse individuals.

Responsibilities

• Conduct intake needs assessments for housing services.
• Create individualized service plans and document client services and outcomes.
• Conduct crisis intervention as needed.
• Assist clients with documentation for benefits and housing applications.
• Coordinate with property management and landlords to mediate issues.
• Travel across the five boroughs to serve participants.
• Maintain accurate documentation and outcomes data.
• Perform other duties as assigned.
