Responsibilities

• Conduct intake and develop ISP addressing housing goals and income goals.
• Provide referrals and linkages to resources to achieve ISP goals.
• Complete program-specific assessments (e.g. VI-SPDAT).
• Evaluate strengths and challenges and conduct 90-day reviews with participants.
• Maintain participant files per PATH policies, HIPAA, and documentation standards.
• Provide crisis intervention and trauma-informed care to increase safety.
