Job summary

PATH is seeking a Case Manager to support clients experiencing homelessness in San Diego, applying a compassionate, "whatever it takes" approach across interim housing and housing placement.

This full-time, non-exempt role requires driving and a commitment to assisting with healthcare, social services, and CES navigation, with opportunities for growth across California.

Responsibilities

• Conduct intake and develop ISP addressing housing goals and income.
• Provide referrals and support to resources for ISP goals.
• Complete program assessments (e.g. VI-SPDAT).
• Evaluate strengths and plan 90-day reviews with participants.
