Job summary

Volunteers of America, Utah seeks a Housing Case Manager to guide clients from the Geraldine E. King Women's Resource Center toward stable housing. You will provide direct case management, crisis support, and goal-oriented planning, leveraging trauma-informed approaches and extensive community resources.

You will coordinate services, attend weekly meetings, and maintain thorough records to support successful client outcomes and rapid housing transitions.

Qualifications

• Must qualify for Utah DHS Case Management Certification within 60 days of hire.
• GED or High School Diploma required for Utah Case Management Certification.
• Experience in social services or related field preferred.

Responsibilities

• Assist clients in developing goals/service plans to overcome homelessness and secure housing.
• Participate in clinical case reviews and coordinate referrals to appropriate providers.
• Provide trauma-informed case management including crisis intervention and ongoing assessments.
• Identify priority needs and coordinate services accordingly.
• Attend and contribute to weekly staff/clinical meetings.
• Maintain records and participate in research or special projects as needed.
• Use agency vehicles to provide transportation.
• Ensure on-site work to deliver direct client care.
