Job summary

Touchstone Mental Health in Minneapolis seeks a full-time Housing Case Manager to support residents at Lydia Apartments. You will provide comprehensive case management, advocacy, and connection to community resources to promote housing placement and retention.

The role requires a valid driver's license and a clean driving record; prior experience with homelessness and with mental health or chemical dependency is highly desired.

Qualifications

• Provide comprehensive case management services to residents.
• Identify and address barriers to housing placement and retention.
• Conduct assessments to determine additional support needs.
• Connect clients with community resources and programs.
• Valid driver's license, vehicle insurance, and driving record required.
• Experience in housing case management and homelessness is desirable.
• Experience with mental health and/or chemical dependency issues is desirable.

Responsibilities

• Maintain a work schedule of 40 hours per week, Monday-Friday daytime hours.
• Provide housing case management and advocacy for Lydia Apartments residents.
• Collaborate with individuals to address barriers to housing placement and retention.
• Conduct assessments to determine additional support needs related to housing.
• Promote long-term housing success by linking clients to community resources and services.
