Job summary

Residential Transitions, INC. is seeking a Targeted Case Manager to assess needs, plan services, and monitor progress for clients with serious and persistent mental illness and co-occurring disorders. You will link individuals to housing and community resources, maintain records, and support self-directed living.

The role requires Minnesota-licensed pathways, including a bachelor’s in behavioral science or equivalent and/or CMA/RN experience with SPMI training. Driver’s license is required.

Qualifications

• Bachelor’s degree or equivalent in behavioral science or related field.
• Meets MN definition of Mental Health Practitioner.
• If no behavioral-science degree, four years CMA experience under supervision OR RN with psychiatric training plus three years MH experience.
• Other acceptable pathways include DHS waiver qualification or 80 hours SPMI training.
• Demonstrated competency in serving adults with serious and persistent mental illness (SPMI).
• Must have valid driver’s license and acceptable driving record.

Responsibilities

• Assess needs, identify goals, and plan services for individuals with SMI, SUD, and co-occurring conditions.
• Coordinate services and monitor progress; ensure documentation and records meet standards.
• Provide flexible, individualized support to help individuals lead self-directed lives.
• Access community and housing resources aligned to client needs and preferences.
• Facilitate communication among care team to align service direction and goals.
• Provide crisis assessment and intervention as needed.
• Promote inclusive, culturally competent workplace.
• Meet with adults at least every 6 months or as needed; coordinate services.

CMA experience (4 years under supervision)

RN with psychiatric training + 3 years MH experience

DHS waiver-qualified as case manager (1998)

SPMI training (80 hours)
