Job summary

Avivo is launching Avivo Village Central Minnesota and seeks a Case Manager, Housing (Site Based) in Saint Cloud, MN. You will help residents set housing and wellness goals, connect to services, and support independence within a trauma-informed framework.

This role offers a 32-hour work week with full-time pay and a comprehensive benefits package; experience with homelessness and housing stability is valued.

Qualifications

• High school diploma or GED required or equivalent.
• Experience with individuals experiencing homelessness or related populations preferred.
• Associate degree or higher may substitute for some experience.

Responsibilities

• Build trusting, person-centered relationships with residents.
• Complete assessments and develop individualized housing and wellness goals.
• Provide ongoing case management focused on housing stability and independence.
• Coordinate services with internal teams, landlords, and partners.
• Maintain timely documentation in electronic systems.
