Job summary

Avenue 360 Health and Wellness in Houston seeks a Continuum of Care Housing Case Manager to deliver case management and supportive housing services. You will work with participants to ensure housing stability, coordinate with partners, and manage documentation for program compliance.

Responsibilities include assessing eligibility, assisting with housing searches, and maintaining accurate records while coordinating with property owners and community resources.

Qualifications

  • Bachelor’s degree or commensurate experience; social work background preferred.
  • At least 2 years of case management or customer service experience.
  • 2 years of housing experience with local, state, and/or federal programs preferred.
  • 2 years of experience working with individuals and/or families experiencing homelessness preferred.

  • Assist participants in securing housing based on household size and preferences.

  • Develop relationships with property owners and managers.
  • Educate clients on Fair Housing laws, tenant rights, and responsibilities.

Role Overview

The Continuum of Care (CoC) Housing Case Manager provides permanent supportive housing program participants with case management and supportive services to help maintain housing stability and prevent the occurrence or recurrence of homelessness. This role ensures high‑quality service delivery, accurate documentation, and strong coordination with internal teams, property owners, and community partners.

Areas of Accountability
Case Management & Participant Support
  • Assess participants’ eligibility and program appropriateness; obtain required enrollment documentation.
  • Accepts Coordinated Access System referrals as assigned and follows established protocols to enroll clients into housing programs.
  • Assist participants in securing appropriate housing based on household size, composition, geographic preference, and other determinants.
  • Provide case management through on‑site visits, phone consultations, and home visits.
  • Develop individualized service plans with participant input addressing residential stability, income, education, and health‑related goals.
  • Demonstrate knowledge of community resources and coordinate linkages to services.
Housing Program Operations
  • Develop and maintain strong working relationships with property owners and property management.
  • Assist clients with the housing search and rental application process.
  • Educate clients on Fair Housing laws, tenant rights, and tenant responsibilities.
  • Determine rent subsidy amounts in accordance with federal guidelines.
  • Perform Housing Quality Standards (HQS) inspections prior to move‑in and annually.
  • Secure all required documentation (e.g., W‑9, lease agreements, identification documents).
  • Prepare and submit monthly rent and utility assistance check requests in a timely manner.
  • Maintain professional objectivity in emotionally charged situations.
  • Perform other duties as assigned.
Data Management & Compliance
  • Maintain all data management requirements within the Homeless Management Information System (HMIS).
  • Enter complete and accurate client information in compliance with agency protocols.
Education, Licensure/Certification Requirements
  • Bachelor’s degree or commensurate experience; undergraduate degree in social work or closely related field preferred.
  • At least 2 years of case management or customer service experience required.
  • 2 years of recent housing experience with local, state, and/or federal programs preferred.
  • 2 years of experience working with individuals and/or families experiencing homelessness preferred.
